I've had a few occurances where Media Browser gives a blank episode thumbnail or banner(?) (Not quite sure of the proper terminology).
In some cases, it appears to have been a download problem with the image file - a .jpg exists but it's zero size so nothing to display.
In the other form of the problem, there is no filename tag in the metadata but MCM never again tries to fetch the graphic, presumably as the metadata file already exists?
Neither of these conditions seem to give the yellow 'incomplete' warning in the media list & the TV Episode auto-scan does not attempt to fetch the missing graphics.
Is there some way of marking these files with incomplete data for a later re-try, or possibly auto reprocess if there are more metadata files than (none zero size) episode thumbnails?

I have a related issue, MCM is deleting episode thumbnails created by other applications.
